From time to time we will hand out an alcohol consumption questionnaire to all patients attending for an appointment. This should be completed in the waiting room and taken with you to your appointment. We do this from time to time to monitor patient's alcohol consumption. This can vary depending on your circumstances so even if you have filled one out in the past please complete one if asked as it will allow your medical records to be updated.
If you express any concerns to your GP about alcohol consumption you will be asked to complete one of these forms which will help the GP to decide how to ensure you get any help and support you need.
